Applications should include this cover sheet, a short project narrative, a project summary in lay language that is suitable for public use, the annual budget (no more than $225,000 and a maximum of 20 percent in indirect costs), and a budget narrative. The application in its entirety should not exceed five pages, excluding the cover sheet.
Applicants should also attach curriculum vitae, including research and professional experience. List in chronological order positions held, professional experience, and citations of publications (in chronological order and with full biographic information, including titles, pages, publisher, etc.)
